The best any psychiatrist can do is ask somebody, "Are you sucidal?". There are probably some more refined interviewing techniques that come into play to establish if an inmate might actually off themselves or not, but in the end, if somebody insists they aren't suicidal when you ask them straightforwardly, then you will just have to take their word for it.
The therapist I was seeing for a while told me about one of her patients who went to see her for about a year, and during that time he never even once hinted at any kind of suicidal tendencies. Until the morning he jumped in front of a commuter train.

People who aren't really serious about suicide will often admit to suicidal thoughts in the belief that it'll get them better treatment; people who are determined to kill themselves will often lie about it so that no-one tries to stop them. People with the most severe depressive symptoms tend not to attempt suicide, because they're just too depressed to pick a method, make a plan and carry it out; when someone starts recovering from a severe depressive episode, their risk of suicide tends to increase because they've got more energy to actually carry it out. Conversely, some people appear to be recovering just before a suicide attempt - the relief of thinking that it'll all be over soon makes them seem more cheerful.
We have some standardised tools for assessing suicide risk, but they're hardly better than guesswork - either you miss loads of people who are a high suicide risk, or you identify loads of people who aren't suicidal at all.

Maybe that ties in with the often observed effect that when you first start taking antidepressants, your symptoms can actually worsen before they get better. I've dabbled in antidepressants myself, and I can attest to the fact that what you immediately notice is that the fog of your worst depressive symptoms starts to clear due to the changes in brain chemistry, and even if your type of antidepressant is more a downer, you feel like you are getting energy back. That might cause some people to actually work up the courage to take on offing themselves, but in my case, I kind of went straight from being depressed to feeling like the world wasn't such a horrible place after all.
I'm back on mirtazapine these days, but not for its antidepressant effect, but because I've suffered from insomnia for years, and mirtazapine is well known to have good off-label effectiveness against it. My GP said that because I tolerated mirtazapine very well when I was depressed, there'd be no harm in me trying it for insomnia as well before we'd try out actual insomnia drugs that could have adverse effects. And he was right, it really works wonders.

Fucking hell, this hit me a little bit tonight. There's the similar phenomenon with heroin addicts where relapsing is the most dangerous time for them. I can see how devastating both could be to the loved ones of someone who has been at rock bottom and is just starting to show signs of hope.

I've read a case report of someone killing themselves by stuffing toilet paper down their throat and another of someone using their fingernails to pick a hole through their flesh down to a major artery. I've seen post-mortem photos of someone who partially decapitated himself with a hand saw. If you can't imagine how anyone could do that to themselves, consider yourself lucky.
