"Suicide watch" is mostly bullshit. If they're checking you at fifteen minute intervals, you can hang yourself with time to spare; if you're determined enough to slash your throat down to the carotid artery, you can bleed to death in three minutes. No prison has sufficient resources to watch vulnerable inmates 24/7. I assume Epstein is an intelligent and resourceful chap with a talent for subterfuge; there's no way a mainstream prison unit could have deprived him of the means and opportunity to kill himself.
I'm not ruling out the possibility of a conspiracy, but people top themselves in prisons and psychiatric inpatient units on a daily basis.

These are reddit comments and should be taken with a skipful of salt, but I've seen a couple of people who've claimed to have worked in suicide watch in prisons and they were adamant that it should be impossible. Firstly there are no regular linens allowed - inmates are forced to wear a 'pickle suit', a sort of velcro-tied smock made of plastic padding that cannot be torn. If there's any risk of the inmate trying to bash their head against a wall as a deperate measure, they'll just be strapped to a bed, no matter how distressing they find it. This isn't your average psychiatric ward where patients' comfort and well-being is a concern - the screws just want the person to be unable to kill themselves, end of story.
It's perfectly possible that they just buggered up of course. But while I'm not as conspiratorial as most folk, I still find this all rather dodgy. The fact that with his death, a full federal criminal case will now not be launched is mightily convenient. It isn't a stretch to assume that some very wealthy and powerful people saw his trial as a threat and decided that bumping him off - or engineering a situation where he was free to bump himself off - was the safest course of action.

The best any psychiatrist can do is ask somebody, "Are you sucidal?". There are probably some more refined interviewing techniques that come into play to establish if an inmate might actually off themselves or not, but in the end, if somebody insists they aren't suicidal when you ask them straightforwardly, then you will just have to take their word for it.
The therapist I was seeing for a while told me about one of her patients who went to see her for about a year, and during that time he never even once hinted at any kind of suicidal tendencies. Until the morning he jumped in front of a commuter train.

Maybe that ties in with the often observed effect that when you first start taking antidepressants, your symptoms can actually worsen before they get better. I've dabbled in antidepressants myself, and I can attest to the fact that what you immediately notice is that the fog of your worst depressive symptoms starts to clear due to the changes in brain chemistry, and even if your type of antidepressant is more a downer, you feel like you are getting energy back. That might cause some people to actually work up the courage to take on offing themselves, but in my case, I kind of went straight from being depressed to feeling like the world wasn't such a horrible place after all.
I'm back on mirtazapine these days, but not for its antidepressant effect, but because I've suffered from insomnia for years, and mirtazapine is well known to have good off-label effectiveness against it. My GP said that because I tolerated mirtazapine very well when I was depressed, there'd be no harm in me trying it for insomnia as well before we'd try out actual insomnia drugs that could have adverse effects. And he was right, it really works wonders.
