I feel as though I've learnt this already, but do you just get the one transplant and if that fails you're boned? Or would he just have been back at the bottom of the list?

If you read the article itself, it says that the cause of death is unclear, or at least not yet announced. The first transplant didn't necessarily fail, we don't know that yet.

On the other hand, if a transplant does fail, chances are a second transplant wouldn't last long either.
