I really don't understand how it gets to this point. Like, I realise cocaine corrodes and wears away the tissue, but in my experience, it becomes near impossible to actually inhale the stuff after a certain point, because your sinuses get scabby, swollen, and essentially lock up. Is it more that they rip their own faces up trying to pick at it, so they can snort more?

That soft tissue irritation (combined with stress, poor nutrition etc) make you vulnerable to infection. It's the infection that does the really deep damage. You see the same thing with heroin addicts - there's only so much physical damage you can do to yourself with a 29ga needle, but an abscess will rot a hole down to the bone if your immune system is fucked. In Westbrook's case, the damage has been compounded by failed restorative surgeries and a bout of septicemia.
