Tattoos close to the bone are always more painful than on fat or muscle but the skull is probably less sensitive than your elbow or knee. It'll likely bleed more than normal tattoos. They're not that painful.

While not all that painful it still puts your body under a fair amount of stress. I'd be more worried about eating properly and being reasonably well rested for the appointment than any pain that may be involved.

I have one on my neck and it was bearable, but you need to be aware of exactly what you're in for. The density of nerve cluster on your head is higher than other parts of your body. It'll hurt and you'll feel it in parts of your head that aren't being tattooed. You're in for at least an hour of that.
