Job I have is making me anxious and stressed out.

If I have a good medical reason, and a note backing me up, I can leave the job and sign on.

I'm not sure how I'd approach that with my doc though. Do I tell him the reasons (which are genuine) and then ask for a note at the end, or do I make my end-goal clear?
