If a player has a contract with one club, then he cannot play for another unless he breaks that contract with the other club. For that to be valid, the other club has to agree to make the contract-breaking valid. This is why money is paid.

Whenever you hear of an offer which "triggered a clause in the player's contract", it's almost certainly one along the lines of requiring the club to notify the player if an offer above a certain value is made.
Something that happens more often than perhaps it should is players engaging in subterfuge to try and set up a transfer. Challenging the management, faking injuries, starting rumours, etc. Sometimes the first a club knows about its apparent interest in a player is when it appears in the press.
